Multi Line Service Reuest

How would you go about implementing a mutli line form in FS? We are pushing FS outside of IT with a view towards and enterprise service desk. We require forms like below to be presentable as a service catalog item.

Sorry for missing out this thread.


I’m afraid we don’t support multi-line service requests at the moment. However, I’ll be happy to share this as an enhancement request with our product team.


Meanwhile, a quick suggestion, can we recommend users to download the form by providing a link and then make the attachment mandatory so that they could attach the form which has been downloaded and filled. Let us know your thoughts on this.

Please do add this. I have built a Service Catalog form to 'mimic' this but it is VERY cumbersome and very long. Not the best user experience. Not sure how you would replicate something like this and make it an editable element by Fresh Service Admins though. Not an easy one to do.
